In order to register with us, please either complete the below initial online form, which will send an email to our staff who will contact you to fill out our application pack, or alternatively, you can download and complete the form linked below, which contains our complete application pack.
If you are a returning member of staff who has taken a leave of absence, or a staff member who would like to update contact details, please download and complete the below form. If you would like to update your account details, please contact the office on 01474 520097 to arrange for an account details form to be sent to you. Please note, you will also need to visit the office for a fitness to practice assessment, and to update supporting documents.
Below is a copy of our time sheet for use by our staff. All time sheets can be sent to email@example.com for processing, and must be received by 12pm on Monday to be eligible for Friday payments. Time sheets must be legible, completed in black ink, and must be signed and authorised by a senior member of staff. Time sheets can also be faxed to 01474 520097. Our fax line is open between 9am and 12pm on Mondays. You can also bring time sheets to our office between 9am and 12pm on Mondays.
